In addition to our case management services and collaborative programs, HPC maintains programs to improve the lives of the homeless and near homeless of Lower Cape Cod: (for our fundraisers and events, see our events section)
Staffed with three trained and experienced case managers, the Homeless Prevention Council provides extensive counseling and guidance to those at risk of becoming homeless. The client services include; advocacy and access to housing programs and funding, budgeting and negotiation for rent and mortgage payments. In addition, the case managers assist clients in assessing their ability to earn a living and, if appropriate, encourage them to pursue further job training and/or education.
Homeless Youth Outreach
Homeless Youth Outreach is a new program offered by the Homeless Prevention Council. This program will target and support youth on the Lower Cape in need of stable and safe housing, community resources and a network of support. Through community outreach, Case Managers will coordinate supports with youth groups, associations, schools and other organziations. The long term goal of this program will create connections to resources and community groups while providing comprehensive case management to provide solutions and address long term needs.
This program will serve youth ages 16-24 who may or may not be enrolled in school. For youth under the age of 18, HPC will coordinate case management services in collaboration with the parent or guardian or will refer youth to an appropriate support service for minors.

Backpack-to-School program completes its 15th year.
Local children are supplied with backpacks filled with school required items. Generous donations of cash and product allow HPC to fulfill the local need.
Adopt-A-Family program pairs children who are most in need with people and businesses who want to give them a wonderful holiday. Donors receive the first names of their child/children and their holiday wish list. The donors purchase the requested gifts and deliver them to the HPC office where they are prepared for pick-up by the families who remain anonymous.
